UNDERSTANDING TOTAL LAPAROSCOPIC HYSTERECTOMY
What is Total Laparoscopic Hysterectomy (TLH)?
Total Laparoscopic Hysterectomy (TLH), also called keyhole surgery, is a modern surgical technique of removal of uterus through small incisions (0.5 to 1cms), in contrast to large incisions used in traditional (open) surgery. The surgery is performed under general anaesthesia. Small size of incisions results in reduced post-operative pain, less blood loss, faster recovery and shorter hospital stay.

Which patients need TLH?
In some conditions such as large fibroids, ovarian cysts, tumours, endometriosis etc the removal of uterus through vaginal route is not possible. Here TLH comes as an alternative to abdominal hysterectomy with avoidance of large unsightly incisions. TLH is indicated for the treatment of following diseases-

  1. Excessive / irregular menstrual bleeding not responding to medical treatment.
  2. Uterine fibroids in patients who have completed their families.
  3. Adenomyosis and severe endometriosis.
  4. Cancers of uterus, cervix and ovaries.
  5. Others such as tubo-ovarian masses, PID etc
  Are their any benefits of TLH over open surgery?

 TLH is an alternative to abdominal hysterectomy for the following benefits-

What is the difference between TLH and LAVH?
TLH is a more advanced form of LAVH (Laparoscopic Assisted Vaginal Hysterectomy). In TLH the complete surgery, including the final stitching of vagina is done through the Laparoscopic route. This prevents future sagging of vagina and reduces chances of post operative vaginal discharge.

Is there any risk associated with laparoscopie hysterectomy?
In expert hands the risk is minimal. But as per the world wide statistics, there is a small possibility of mechanical or thermal injury to the bladder, bowel or blood vessels. In most situations, these injuries can be treated laparoscopically.

What is the recovery period following laparoscopic hysterectomy?
There is minimum discomfort after TLH / LAVH. Patients are discharged within 24 to 48 hours of surgery.They may resume light work within 1-2 days & are ready to return to full activity within 15 days of laparoscopy.
